Please read these Terms carefully before using the Integrix Health website or engaging our services.

1. Acceptance of Terms

By accessing or using the Integrix Health website (www.integrix.health), submitting any form, booking a service, enrolling in a program, providing your phone number, or communicating with Integrix Health, LLC in any capacity, you agree to be bound by these Terms and Conditions ("Terms").

If you do not agree to these Terms, please do not use this website or our services.

These Terms are in addition to — and do not replace — any separate service agreements, informed consent documents, or intake forms you may complete as a patient or client of Integrix Health, LLC.

2. About Integrix Health

Integrix Health, LLC is a functional medicine and clinical wellness practice operated by Dr. Paul Bekkum, DC, CCEP, in Moorhead, Minnesota. Our services include:

  • Virtual-first functional medicine consultations and care programs
  • Functional rehabilitation and neuromusculoskeletal assessments
  • StructureIQ™ Performance assessments and corporate wellness services
  • 3x4 Genetics nutrigenomic consulting and interpretation
  • Clinical education, newsletters, and digital health content
  • The Integrix Method clinician training program

Dr. Bekkum is a licensed Doctor of Chiropractic and Certified Chiropractic Extremity Practitioner (CCEP). Services are delivered consistent with the scope of practice under Minnesota and North Dakota law.

3. Not Medical Advice

Important: The content on this website — including articles, assessments, videos, guides, and program descriptions — is provided for informational and educational purposes only. It does not constitute medical advice, diagnosis, or treatment.

No patient-provider relationship is created by visiting this website, submitting a contact form, downloading a resource, or booking a Discovery Call unless and until a formal clinical intake has been completed and an active care relationship has been established with Integrix Health, LLC.

Always consult with a qualified healthcare provider before making changes to your health, medications, supplementation, or exercise regimen. Do not delay seeking professional medical attention based on information found on this website.

4. Telehealth & Virtual Services

Integrix Health, LLC delivers clinical services via telehealth through the Practice Better platform. By engaging telehealth services, you acknowledge and agree that:

  • Telehealth is subject to applicable Minnesota and North Dakota laws governing virtual care
  • Technology limitations (internet connectivity, video quality, platform outages) may occasionally affect service delivery; Integrix Health is not liable for service disruptions caused by third-party platforms or infrastructure
  • You are responsible for ensuring you are located in a state or jurisdiction where Dr. Bekkum is authorized to provide care at the time of your appointment
  • Telehealth sessions may not be appropriate for all clinical presentations; Dr. Bekkum reserves the right to refer you to in-person or emergency care when clinically indicated
  • You must have completed a valid informed consent for telehealth services before your first clinical session

5. SMS & Text Messaging

5.1 Consent to Receive Text Messages

By providing your mobile phone number to Integrix Health, LLC — whether through a website form, intake questionnaire, opt-in checkbox, or verbal authorization — you expressly consent to receive text messages (SMS/MMS) from Integrix Health, LLC at the number provided.

This consent is not a condition of purchasing any service. You may opt in to text messaging separately from any clinical enrollment.

5.2 Types of Text Messages We Send

Depending on your relationship with Integrix Health, you may receive the following categories of text messages:

SMS Program Disclosure

  • Program name: Integrix Health Text Updates
  • Sender: Integrix Health, LLC — Dr. Paul Bekkum, DC, CCEP
  • Message frequency: Varies. Appointment-related messages are sent as needed; marketing messages are sent no more than 4 times per month.
  • Message & data rates may apply. Check with your wireless carrier for your plan's messaging rates.
  • To opt out: Reply STOP at any time to unsubscribe from all text messages. You will receive one confirmation message, and no further messages will be sent.
  • To get help: Reply HELP for assistance, or contact us at [email protected] or 701-347-1968.
  • Supported carriers: AT&T, Verizon, T-Mobile, Sprint, and most major U.S. carriers. Carrier support may vary.

5.3 HIPAA & PHI Limitations

Standard SMS is not a HIPAA-secure channel. We do not transmit Protected Health Information (PHI) via standard text message. Appointment reminders and administrative messages will not include clinical details, diagnoses, lab values, or treatment information. Secure clinical communication occurs exclusively within the Practice Better patient portal.

If you have questions about your health information, please contact us through the Practice Better portal or by calling our office directly.

5.4 Opting Out

You may withdraw consent to receive text messages at any time by:

  • Replying STOP to any text message from Integrix Health
  • Emailing [email protected] with your name and phone number and a request to be removed
  • Calling 701-347-1968 during business hours

Opting out of marketing texts does not affect appointment reminders associated with an active care relationship unless you specifically request removal from all communications.

5.5 TCPA Compliance

Our text messaging practices comply with the Telephone Consumer Protection Act (TCPA) and applicable FCC regulations. We obtain express written consent prior to sending marketing text messages and honor all opt-out requests promptly. We do not use autodialed or pre-recorded voice messages without prior express consent.

6. Website Use

You agree to use this website only for lawful purposes and in a manner that does not infringe the rights of others. You shall not:

  • Attempt to gain unauthorized access to any portion of this website or its underlying systems
  • Use automated scraping, crawling, or data harvesting tools without express written permission
  • Submit false, misleading, or fraudulent information through any form
  • Transmit malware, viruses, or any other harmful code
  • Impersonate any person or entity, or misrepresent your affiliation with any person or entity
  • Reproduce, republish, or commercially exploit any content from this website without prior written consent

Integrix Health reserves the right to restrict or terminate access to this website for any user who violates these Terms.

7. User Accounts & Forms

Certain services require you to submit personal information through intake forms, assessment tools, or scheduling systems. By submitting such information, you represent that:

  • The information you provide is accurate, current, and complete
  • You are at least 18 years of age
  • You have the legal authority to enter into these Terms

You are responsible for maintaining the confidentiality of any portal credentials associated with your patient account on Practice Better. Notify us immediately of any unauthorized access to your account.

8. Payments, Fees & Refunds

8.1 Service Fees

All service fees are disclosed at the time of enrollment or booking. Payment is required in advance unless alternative arrangements have been confirmed in writing by Dr. Bekkum.

8.2 Cancellations & No-Shows

We require a minimum of 24 hours' notice to cancel or reschedule an appointment. Cancellations made with less than 24 hours' notice, and missed appointments without notice, may be subject to a cancellation fee equal to the full session cost. Discovery Calls are exempt from cancellation fees when cancelled at least 2 hours in advance.

8.3 Program Refunds

Multi-session program fees are generally non-refundable once clinical care has commenced. Refund eligibility is assessed on a case-by-case basis at Dr. Bekkum's sole discretion. Approved refunds will be issued within 10 business days via the original payment method.

8.4 Insurance

Integrix Health, LLC does not currently bill insurance for functional medicine or wellness services. All fees are out-of-pocket ("self-pay"). We will provide a superbill upon request, which you may submit to your insurer for potential out-of-network reimbursement — we make no guarantee of reimbursement.

9. Intellectual Property

All content on this website — including text, graphics, logos, photographs, videos, assessments, guides, frameworks (including the Integrix Method™, StructureIQ™, and Neuroplanar Method®), and clinical education materials — is the exclusive intellectual property of Integrix Health, LLC or its licensors.

You may not reproduce, distribute, modify, create derivative works of, publicly display, or commercially exploit any content from this website without prior express written consent from Integrix Health, LLC.

Limited personal, non-commercial use (e.g., printing a page for your own reference) is permitted provided you retain all copyright and proprietary notices.

10. Third-Party Platforms

Integrix Health uses third-party platforms to deliver services, including but not limited to Practice Better (telehealth and clinical records), Fullscript (supplement dispensing), 3x4 Genetics (genetic panel processing), and Stripe (payment processing). Use of these platforms is subject to their own terms and privacy policies, which we encourage you to review.

Links from this website to third-party sites are provided for convenience only. Integrix Health does not endorse, control, or assume responsibility for the content or practices of any third-party website.

11. Disclaimer of Warranties

This website and all content, services, and materials are provided on an "as is" and "as available" basis without warranties of any kind, either express or implied, including but not limited to warranties of merchantability, fitness for a particular purpose, or non-infringement.

Integrix Health does not warrant that the website will be uninterrupted, error-free, or free of viruses or other harmful components. We do not warrant the accuracy, completeness, or currency of any educational content published on this website.

12. Limitation of Liability

To the fullest extent permitted by applicable law, Integrix Health, LLC, its officers, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of this website or our services, even if Integrix Health has been advised of the possibility of such damages.

Our total aggregate liability to you for any claim arising under these Terms shall not exceed the total fees paid by you to Integrix Health in the 12 months preceding the claim.

Nothing in these Terms limits liability for gross negligence, willful misconduct, or any liability that cannot be excluded by law.

13. Indemnification

You agree to indemnify, defend, and hold harmless Integrix Health, LLC and its officers, employees, and agents from and against any claims, damages, liabilities, costs, and expenses (including reasonable attorneys' fees) arising from: (a) your use of this website or services; (b) your violation of these Terms; or (c) your infringement of any third-party rights.

14. Governing Law & Disputes

These Terms are governed by the laws of the State of Minnesota, without regard to its conflict of law principles. Any dispute arising out of or relating to these Terms or the services provided by Integrix Health, LLC shall be resolved as follows:

  1. Informal Resolution: The parties will first attempt to resolve any dispute informally by contacting Integrix Health at [email protected]. We will attempt to resolve disputes within 30 days of notice.
  2. Mediation: If informal resolution is unsuccessful, the parties agree to submit the dispute to non-binding mediation in Moorhead, Minnesota before initiating formal legal proceedings.
  3. Jurisdiction: Any legal action shall be brought exclusively in the state or federal courts located in Clay County, Minnesota.

Nothing in this section prevents either party from seeking injunctive or other equitable relief for intellectual property violations.

15. Changes to These Terms

Integrix Health, LLC reserves the right to update these Terms at any time. Material changes will be communicated via:

  • A prominent notice on integrix.health
  • Email notification to active patients and enrolled program participants

The "Last Updated" date at the top of this document reflects the most recent revision. Continued use of this website or our services after any revision constitutes your acceptance of the updated Terms.
