What this site collects

This website does not host forms. Booking, questionnaires, and clinical records are handled inside our practice-management platform, which is covered by a Business Associate Agreement. This site uses Google Analytics and Google Tag Manager to measure page traffic; no health information is transmitted through them.

Genetic and health information

Written consent for the collection, storage, and use of genetic information is obtained separately, before any test is ordered. On request after your program ends, raw genetic data files can be deleted from active practice systems; the record of your work with us, and the interpretations made from that data, are retained as required by applicable record-keeping obligations. The consent document sets out these terms in full.
