The Symptom Burden Score — the routing instrument
The Medical Symptom Questionnaire (MSQ) scores symptoms across fifteen body systems — head, eyes, ears, nose, mouth and throat, skin, heart, lungs, digestion, joints and muscles, weight, energy, mind, emotions, and other. The total is not a diagnosis. It is a measure of how much your body is carrying right now, and it decides where you begin:
- A high score means there is active symptom burden to bring down before anything else. Conventional labs are ordered, and the work is Relief and Rehabilitation.
- A moderate score means you are functional but not optimal. This is Vitality by Design™ territory.
- A low score means you are relatively well and the question shifts from "what is wrong" to "how fast am I aging, and can I slow it." That is Longevity by Design, and it starts with genetics.
The score is repeated on a schedule throughout care. It is the number that tells us whether the plan is working — and, after care ends, the early-warning signal if something starts to drift.

Genetics — the fixed layer
A one-time genotype (3X4 Genetics) reports variants across pathways that matter for everyday biology: inflammation, detoxification, methylation, blood-sugar handling, lipid metabolism, oxidative stress, neurotransmitter processing, vitamin handling, and circadian rhythm.
A gene is a predisposition, not a destiny. Genetic susceptibility only matters when it is being expressed — which is why the genotype is never read alone. It is cross-referenced against your labs and your symptom score before anything is recommended.
Genetics are not part of the entry process. They are used inside Longevity by Design, where they do the most work.
Biological age — the moving layer
Chronological age is a count of birthdays. Biological age is an estimate of how old your cells are behaving, derived from DNA methylation patterns. Epigenetic testing (TruDiagnostic TruAge and TruHealth) reports two things we care about:
- Biological age — a snapshot of where you are today.
- Pace of aging — how fast the clock is currently moving, which responds to lifestyle change on a timescale of months.
Pace of aging is the tracking metric in Longevity by Design. Symptom scores eventually reach zero and go quiet; pace of aging keeps moving, which is what makes it useful once you are well.
Seven systems, plain language
Every finding is organized under one of seven physiological systems, so a plan stays coherent instead of becoming a list of supplements:

Food first — the order of operations
For any system that needs work, interventions are applied in a fixed order. Supplements are sixth on the list, not first.
